Saratoga Springs, New York, August 2004 – Present. Founder and partner. Responsible for overseeing recruitment and screening activities of respite workers providing planned respite and skill building to disabled individuals in the Capital District. Such activities include recruitment of college students for internships, conducting trainings, orientation and screening of potential applicants, adherence to policies and procedures in performing respite and completion and submission of required reports to subcontracting agencies.

York, Jan 2004 – Present. Responsible for the individualized service environment, advocating for the individual and the family, developing relationships with community service providers and acting as a liaison between the individual and the service providers. Other duties include:
  • Attend school district CSE annual and subcommittee reviews 
  • Attend at least 15 hours of Medicaid Service Coordination trainings yearly
  • Meet with clients at least one time monthly 
  • Review and update the Individualized Service Plan biannually
  • Maintenance of a complete individual file 
     
Reality Check Coordinator (Youth Empowerment), Catholic Charities
Amsterdam, NY, July 2001 – December 2003. Responsible for the development and implementation of a youth driven grassroots campaign. Other duties included:
  • Managing line items in a grant

Provided support to emotionally disturbed and abused youth afflicted with the following disorders: bipolar disorder, schizophrenia, conduct disorder, borderline personality disorder, narcissistic personality disorder, post traumatic stress disorder, depression etc. Other duties included:
  • Accompany youth to psychiatric appointments and group counseling sessions providing input as requested from clinicians.
  • Educating youth in identified deficit areas (hygiene, cooking, cleaning, self-esteem, safety etc).
  • Ability to diffuse a crisis without utilizing physical intervention techniques
  • CPR and First Aid Certification

  • Facilitated group meetings for the youth in the residence
  • Wrote progress notes: goal areas, service provider appointments, crisis situations and to report any discussion with resident.
  • Included youth from the residence in Reality Check activities, which enhanced socialization skills.

York, Feb 2000 – March 2001. Responsible for the development and implementation of a functional assessment. Provided quality assurance in residential and day program settings. Kept advocates and family members informed. Other duties included:
